UGLY Design Co. — photo & project sharing choices Version 2026-10-27.1 This form is for adults age 18 or older. Your choices are voluntary. Saying no, leaving every use unchecked, or later changing your mind will not affect your order, price, or service. Only the project and images described in this record are covered. If you choose to allow a use, you give UGLY Design Co. permission to show those images for that selected use: its website/portfolio, its organic social media posts, and/or its paid advertising. These are separate choices. An unchecked use is not permitted by this form. No payment for promotional use is promised. This is a limited, non-exclusive permission, not an assignment of ownership or a blanket release of claims. You must be the adult pictured; the parent or legal guardian authorized to decide for the minor pictured; or the owner/authorized representative of a project with no identifiable people. Buying or commissioning an item alone does not let you authorize another person's likeness. Each other identifiable adult needs to make their own choice. Do not enter a child's name, birthdate, or other identifying details. Do not upload any photos here. This form does not clear copyright or other third-party rights in a source photograph, artwork, logo, or design. UGLY Design Co. must separately confirm those rights and the signer's authority before using an image. Submission is not identity verification. Every record starts pending manual review; nothing is automatically published. Your naming choice applies only to you or an authorized business. No child's name will be published based on this form. Unless you expressly choose an adult first-name or business credit and provide it, no name or account tag is permitted. UGLY Design Co. may resize, crop, or adjust light/color for the selected display, but not materially misrepresent the subject or use the image for an unrelated purpose. To change your choices, withdraw permission for future use, correct this record, request a copy, or ask about deletion, contact udcsigns@gmail.com and include your receipt ID or project reference. UGLY Design Co. will confirm the request with you before changing the record. Withdrawal covers future new uses and removal from channels UGLY Design Co. controls where reasonably possible. Copies already shared by other people, previously distributed material, or third-party caches may remain. Your original choice and any withdrawal can be retained as a record of what was authorized; keeping a record does not authorize new use. Your adult name, contact email, role, project description, choices, typed signature, form version, and submission time are stored privately for UGLY Design Co. to review and document your instructions, not displayed in the public gallery or added to a marketing mailing list. The site uses its hosting/database providers to process and store the record. UGLY Design Co. keeps records while reasonably needed to document an authorized use or resolve a related question, reviews retained records, and can delete them when no longer needed, subject to applicable recordkeeping obligations. This form does not request photos, government ID, child names, or birthdates. Do not add sensitive personal details to the project description. Typing your name and checking the confirmations means you intend to sign this particular record electronically and agree to its electronic storage. After a successful save you can download or print a receipt using a current browser; save it before closing the page.
